Required Enrollment Documents
When you register your child, you will need the following:
- Birth Certificate or affidavit explaining the inability to provide a copy of the birth certificate. A.R.S. §15-828.
- Proof of immunizations (your child will not be admitted unless their immunizations are up-to-date).
- Name/Address of previous school.
- Proof of legal custody may also be required.
- Parent/Guardian picture ID.
- Proof of residence.
